Preface
All around the world in our everyday life,we are taking dentistry to a new direction by freeing the patients from their dilemma and ailments.Many stories on this dental theme will arise with that mentioned on a daily basis.This is only a fraction of all of those stories but I want to address here that the responsibility and interest to understand the patient's need is an endless one.We technicians must share concerns and complexes with patients so that we can apply it to our best interest in our work.Not only that,I myself have plenty of behind the scene episodes to share in the process.

In dentistry there is no correct answer or one formula that would be appropriate for all clinical cases.If there was an answer that would mean that all the individual patients will be satisfied with the result.Then additionally,we must determine whether the treatment method was the best option for them.Keep in mind that we cannot always select the best or the ideal treatment.In an actual clinical case,majority of the time the treatment options that technicians work with are very restricted.But even with the option limitation,we must consider to strive for the optimal result for the patient.In order to achieve the optimal result,technicians must reevaluate the diagnosis and the treatment plan; and communicate with both the dentist and the patient until the end of the treatment to achieve the best outcome.There are many points where I learn from patients throughout this process and I can feel my own progress from it.

I have recorded a mere fragment of my six years worth of clinical results from the year 2005-2010 in this book.After reviewing all of these cases,even I noticed some changes in my own work during this period.This improvement is sure enough the result of continuing diligent studies and through my disciplined clinical experience.Additionally,surrounding environment and encounters with various individuals is also an asset to the change.Since the start of the 21st century,digital technology has drastically improved and many benefits are seen in dentistry these days.This is noticeable in my own clinical cases,and many of the digital technological elements are a blessing for dentistry.Take a look at the clinical photos in this book as an example.Majority of these are digital photos that are recorded permanently with no concerns of it fading away over time.2005-2010 was my transitional period from analog(film) to digital photography and if I wrote this book in the future the audience may have felt some reminiscence of technology history.

Patient's past & future.What is important here is that the pre-op and the post-op is not an extreme change.What we want is a natural looking healthy post-op results.However it is a dramatic change filled with excitement from the patient's point of view nonetheless. When we achieve such treatment results,the patient will value our hard work and will return us their very best smile.This is the moment when we feel most rewarded for our work.It is an irreplaceable moment of satisfaction.I feel what's important in our future is to concentrate and strive for the bright future rather than reflecting on the past.

I feel grateful if you all would take out more than just the clinical results and the look beyond the beautiful smile after reading this book; I hope the behind the scene events and stories are picked up through the patient's confident facial expression as well.

 全世界で,毎日多くの患者が歯科医院を訪れ,彼らが持つ不安や疾患,コンプレックスなどを取り除くべく,われわれが携わる歯科医療は進んでゆく.そして歯科医療という舞台の中で,多くのストーリーが生まれている.その中のほんのわずかな症例を筆者も担当し,患者の持つ不安やコンプレックスに立ち向かう責任と興味は尽きることはない.そして,それらの背景には語るべきエピソードとして充分すぎるほどのストーリーが存在している.
 審美修復を行うにあたって,どの症例にも当てはまるような公式や正解を求めても,そんなものは何処にもあるはずがない.もし答えがあるとすれば,それは個々の患者一人ひとりに行った治療の結果が患者自身に心から受け入れられ,そして彼らにとってそれが最良の結果であったのかどうか,ということになるであろう.われわれは,常に理想的な治療を行えるわけではなく,ほとんどの場合は,限られた治療オプションの中で治療を進めることになる.しかし,その限られた範囲の中であっても,常に個々の患者にとって最良の治療結果を提供することを念頭に置かなくてはならない.筆者自身においては,毎日の臨床の中で最良な結果を出すために,診査・診断および治療計画立案から始まり,治療終了までの間に,歯科医師,患者とのコミュニケーションを少しでも良好に行うことを心がけている.そこからは,筆者自身が学ぶことが多々あり,成長させてもらう日々の連続である.
 本書には,2005年から2010年までの間に筆者が行った臨床技工の結果の一部を収録した.本書の制作を進めるなかで,筆者自身の臨床技工が大きく変化していることに気づいた.つまり,歯科医療・歯科技工における経験と知識,そして技術が,ほんのわずかずつではあるが向上していると思えたこと──これは,筆者を取り巻く環境の変化や,さまざまな人びととの出会いが,そうさせてくれたのであろう.また,21世紀以降,世の中では凄まじい勢いで進化するデジタル技術が本格的に導入され,歯科分野においてもデジタルの恩恵に与る部分が多い.筆者も例外ではなく,日々の臨床技工においてもその要素は多分に取り入れられている.本書に掲載した写真一つをとっても,そのほとんどが鮮明なデジタル写真であり,これは色褪せることのない日々の臨床技工の記録となる.2005年以降の6年間は,筆者の写真撮影が本格的にアナログ(フィルム)からデジタルへ置換されていった期間であり,もし仮に未来に本書を開いたならば,まさにその過渡期における時代の変化を感じ取ることもできるのではないだろうか.
 患者の過去と未来.治療前と治療後の結果が,目立ち過ぎた変化ではなく,健康的で自然であること.それでいて,患者本人はドラマティックな変貌を遂げ,胸の高鳴りと興奮が湧き出てくる──そんな治療結果を残せた時は,患者は最高の笑顔でわれわれの仕事を評価してくれる.それは,術者として多くの困難と努力が報われる瞬間であり,何ものにも代え難い喜びとなろう.歯科業界の未来においても,過ぎ去った過去よりも光ある未来を注視し,それに向かって努力していくことが重要であると考える.先を見据えて前進する時,筆者はいつでも「 過去<未来」 とすべく,歩みを進めていこうと思っている.

There has been always something that attracted me to Japanese art and design.Why does it appeal to me so much? I think,because it emphasize timeless,serene simplicity and provide an oasis of philosophical and contemplative calm in a world of strife and Chaos.

There is a Japanese word “ Okuyukashii”. This word contains nuances translated as “sensitive” and “subtle”. When I looked at this book I experienced the feeling of “Okuyukashii” because it demonstrates Japanese spirituality and culture in a skillful way and with a delicate sensibility. Taking this in consideration and knowing about the accuracy,precision,and determination to details of the Japanese masters,makes me feel up most Pleased and honored to write the foreword for Naoki's book.

He is considered to be one of the top dental technologists around the globe.To the best of my knowledge,the Japanese masters are a reference in the world for tooth morphology and nature emulation.I believe that this is the essence to integrate any restoration into a natural smile.

The reason for this testimonial is not only my appreciation of the book,but my respect for its author and his accomplishments.I have been following Naoki over the past decade and his art,sensibility,and creativity to emulate the anterior dentition make him unique and the work speaks foritself.Naoki has the sense of modeling with light,he is an outstanding Photographer,and masters his material and techniques with the highest knowledge and Sensibility.His passion cannot be explained through rules and recipes,instead with feeling.If I could describe Naoki's work in few words,I would say: delicate,sensible,and subtle.

In the fall of 2007,I had the opportunity to meet master ceramist and photographer Naoki Hayashi while attending the Hawaii Mid Pacific Session in Honolulu,a conference for Japanese dental professionals.I was mesmerized with the incredible skill,unvarying quality,and unbelievably naturallooking restorat ions being presented by Mr.Hayashi in his multimedia presentation,which he choreographed with his outstanding photography.Imaginative,provocative,and often surreal,the imagery-which showcased portraits of teeth,smiles,faces,people,and places that surround us everyday-made me feel,contemplate,and dream of what was possible.

Such is the effect of Naoki Hayashi's work.

Since then,I have worked closely with Mr.Hayashi in my practice ,co-diagnosing and planning treatment for aesthetically challenging cases:designing tooth contours,planning arrangement,strategizing optimal tissue scallop form,selecting restorative materials,charting preparation design,and ensuring proper occlusion.
